Professional Services Agreement
By scheduling and attending therapy services with Resiliency Counseling Group LLC, you agree to the following terms and conditions.
Appointment Policy
Sessions are typically 50-60 minutes in length. Please arrive on time as late arrivals will result in shortened sessions.
Cancellation Policy
We require 24 hours notice for cancellations or rescheduling. Late cancellations or no-shows may be charged the full session fee. Exceptions may be made for emergencies.
Payment Terms
Payment is due at the time of service unless other arrangements have been made. We accept:
- Most major insurance plans
- Credit and debit cards
- HSA/FSA cards
- Cash
Confidentiality
All information shared in therapy is confidential, with the following exceptions:
- Risk of harm to self or others
- Suspected child or elder abuse
- Court order or subpoena
- Client authorization for specific disclosures
Emergency Situations
If you are experiencing a mental health emergency, please call 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room. Our practice is not equipped to provide 24/7 emergency services.
Telehealth Services
Telehealth sessions are conducted via secure, HIPAA-compliant platforms. You are responsible for ensuring you have a private, quiet space and reliable internet connection for sessions.
